HERO Substance Use Evaluator
Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty. Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.
Purpose: The Substance Use Evaluator conducts
comprehensive assessments to evaluate individuals for substance use disorders.
This role involves utilizing clinical skills and evidence-based practices to
provide accurate diagnoses, develop treatment plans, and make appropriate
referrals. The evaluator works collaboratively with clients, families, and
multidisciplinary teams to support recovery and promote overall well-being.
Key Responsibilities:
- Conduct
thorough assessments of clients’ substance use history, psychosocial
background, and overall mental health.
- Utilize
standardized assessment tools and diagnostic criteria to evaluate the
severity of substance use disorders.
- Develop
individualized treatment plans based on assessment findings and client
needs.
- Collaborate
with healthcare providers, therapists, and community resources to
coordinate care and support services.
- Provide
psychoeducation to clients and families about substance use disorders and
treatment options.
- Monitor
and document client progress throughout the treatment process, adjusting
plans as necessary.
- Maintain
compliance with state and federal regulations regarding confidentiality
and ethical practices.
- Participate
in ongoing training and professional development to stay current with best
practices in substance use treatment.
- Contribute
to program evaluation and quality improvement initiatives.
- Other
job duties as assigned.
Qualifications:
- Associate’s
degree in chemical dependency counseling, or a related field
- Relevant
certifications of LADC are a must.
- Experience
in substance use assessment and treatment.
- Strong
knowledge of substance use disorders, evidence-based treatment modalities,
and community resources.
- Excellent
communication, interpersonal, and organizational skills.
- Ability
to work independently and as part of a multidisciplinary team.
- Proficient
in using electronic health record (EHR) systems and other technology
tools.
Working Conditions:
- Primarily
office-based with occasional travel for client outreach and community
engagement.
- May
require evening or weekend hours based on client needs.
